Assess your dog...Is your dog ready for the daycare

- Is your dog social,
has been socialized with other dogs off leash?
- Is your dog in good
general health?
- Is your dog current
on the following vaccinations-we will need a copy of current vet
records?
- DHLPP
- Bordetella (every
6 months)
- Rabies
- Flea/Tick/Heartworm
protection
- Has your dog been
spayed or neutered if 6 months or older?
- Is your dog at least
10 weeks old?
- If your dog is recently
adopoted, have you had your dog for at least 30 days?

Fill out required enrollment forms: |

Schedule and attend an evaluation- call us at 317-873-2525
for an appointment. |

- Bring all completed
enrollment forms to evaluation appointment to avoid rescheduling.
We cannot introduce your dog to those at daycare before
we have assessed the information on the forms.
- We will evaluate your
dog's social behavior by observing his/her reaction when safely
introduced to the daycare environment.
- The introduction portion
of the evaluation will take approximately 30 minutes. You will
need to leave the premises during this time as your dog will react
to you being nearby. You might plan to hang out in downtown Zionsville
(2 minutes north of us), at Trader's Point shopping center at
86th and Zionsville Rd, or in the shopping area on Michigan Road
between 96th and 126th.

| 4.
When your dog passes the evaluation... |

- Your dog will be fitted
with a required collar that will be kept in his/her file to wear
during all daycare and lodging visits. This collar is color-coded
by temperment and lets the Dogmates (employees) easily identify
each dog's needs during their stay with us. There is a one-time
fee for the collar (refer
to the Rates page).
- Your dog is now eligible
for daycare, but must complete at least one day of daycare
before lodging overnight.
